Job summary

Qualifications

• Must be willing to perform essential services before, during, and after an emergency.
• Must be able to lift up to 40 pounds.
• Background screening and E-Verify requirements are in place.

Responsibilities

• Manage a caseload of residents and advocate for their well-being and rights.
• Coordinate communication between residents, treatment teams, and guardians.
• Prepare annual and monthly reports for ISP and IDT meetings.
• Monitor Medicaid eligibility and assist with guardian communications.
• Engage with guardians/guardian advocates and document important events.
