Crestwood is seeking an Activities/Behavioral Specialist to provide quality care to residents with behavioral needs and to collaborate across departments to implement and adjust treatment plans. The role supports a therapeutic, homelike environment aligned with our person-centered care model.

Qualifications

• Bachelor's degree or CNA with 2 years mental health experience.
• Experience implementing treatment plans.
• Ability to effectively communicate via written and verbal methods with residents, employees, and outside agencies.

Responsibilities

• Provide quality care to residents with behavioral needs and support care plans.
• Collaborate with other facility departments to provide activities and support and adjust care plans as needed to create a therapeutic, homelike environment.
