Job summary

The Evergreens - Hillview Health Care Campus in La Crosse County is seeking compassionate team members for resident-centered care across AM, PM, and NOC shifts. We offer full-time and irregular part-time schedules with weekend and holiday rotations to support a stable, caring community.

Under supervision of the Assisted Living Supervisor, you will help deliver person-focused care, assist with daily activities, and contribute to a warm, home-like environment for residents.

Qualifications

• Applicants must be at least 18 years of age.

Responsibilities

• Compassionate communication with residents, families, and staff; professional and respectful interactions.
• Observe residents for needs or changes in condition and promptly report concerns.
• Demonstrate dependable attendance and teamwork with supervisors and clinical staff.
• Safely administer medications following the Five Rights and perform basic clinical tasks within scope.
• Assist residents with activities of daily living (ADLs), including hygiene, mobility, meals, and transfers.
• Document care accurately and maintain a safe, clean environment in compliance with regulations.

AVAILABLE POSITIONS

AM SHIFT (6:30 AM TO 3:00 PM)

• Irregular Part-Time (0-39 hours per pay period)

PM SHIFT (2:30 PM TO 11:00 PM)

• Irregular Part-Time (0–39 hours per pay period)
• Part-Time with benefits (64 up to 80 hours per pay period)

NOC SHIFT (10:30 PM TO 7:00 AM)

• Full-Time (80 hours per pay period)

There may also be opportunities to pick up additional shifts as available

All schedules require working every other weekend and participation in a holiday rotation.

Overview

This position provides resident care in the assisted living settings of La Crosse County’s Long-Term Care Campuses, with a strong focus on person-centered care and a home-like environment. Team members are selected by and work under the supervision of the Assisted Living Supervisor.

Key Responsibilities

• Compassionate Communication: Interact with residents, families, and team members with courtesy, professionalism, and respect.
• Good Judgment and Awareness: Observe residents for needs or changes in condition and report concerns promptly.
• Dependable Team Player: Demonstrate reliable attendance, a willingness to learn, and strong collaboration with supervisors and clinical staff.
• Medication and Clinical Support: Safely administer medications following the Five Rights and perform basic clinical tasks within allowed scope.
• Personal Care Assistance: Support residents with activities of daily living (ADLs), including hygiene, mobility, meals, and transfers.
• Documentation and Safety: Accurately document care, comply with assisted living regulations, and maintain a safe, clean environment.

The Evergreens - An exciting new Community Based Residential Facility (CBRF) now open on the Hillview Health Care Campus.
