Evergreen Healthcare Center in Montpelier, OH is seeking a Social Services Designee. You will work with Social Services, Therapy and clinical staff to establish discharge planning and advocate for residents' psychosocial well‑being.

The role requires a high school diploma or GED and prior social services experience in a long‑term care setting, with a focus on compassionate care, confidentiality, and collaboration with caregivers and residents’ families.

Qualifications

• High school graduate or GED required.
• Previous social services experience in a long term care environment preferred.
• Ability to collaborate with social services, therapy and clinical teams.

Responsibilities

• Work with Social Services, Therapy and clinical teams to establish resident discharge planning.
• Build relationships with team and residents to support residents’ quality of life.
• Maintain rapport with direct caregivers to ensure participation in programs for residents’ psychosocial well‑being.
• Report grievances to the Director of Social Services and document appropriately.
