• Social Service designee is required.
• High school graduate or GED equivalent.
• Experience in social services within a long-term care environment.

Responsibilities

• Collaborate with social services, therapy and clinical teams to plan discharge for residents.
• Build rapport with direct caregivers to support programs for residents' psychosocial well-being.
• Report grievances and communicate with the Director of Social Services.
• Advocate for residents, monitor changes and liaise with RN Charge Nurse as needed.
• Provide information to help solve resident problems and enhance services.
