Job summary

Forest Hills Healthcare Center, part of the CommuniCare Family, seeks a Social Services Designee for a full-time day shift. The position offers pay ranging from $20 to $22 per hour and a benefits package including medical, dental, vision, life, LTD/STD, and a 401(k) match, plus daily pay options.

You will develop resident advocacy, coordinate with care staff, report grievances, and improve psychosocial support for residents in a respectful, confidential environment.

Qualifications

• High school graduate or GED equivalent.
• Experience in social services in a long-term care setting.

Responsibilities

• Develop and maintain good rapport with direct caregivers to support residents’ psychosocial wellbeing.
• Report grievances and submit oral/written reports to the Director of Social Services.
• Advocate for residents in collaboration with care staff to meet needs.
• Provide information to the director to help solve resident needs.
• Improve the quality of services and expand activity opportunities.
• Involve family members in activities with the activity department.
• Maintain confidentiality of resident/family information.
• Perform general social services duties.
