Qualifications

Must hold a state license/certification in social work (where practice is required).
Bachelor’s degree in Social Work or related Human Services field.
Minimum of 1 year of social work experience in a healthcare or geriatric setting.
SNF experience preferred; nursing SNF experience a plus.

Responsibilities

Conduct psychosocial assessments and contribute to individualized care plans.
Support residents and families through transitions into long-term care and memory care.
Complete MDS-related documentation accurately and in compliance with regulations.
Advocate for resident rights, dignity, and self-determination.
Coordinate discharge planning and connect residents with community resources.
Collaborate with nursing, administration, and the interdisciplinary team.
Assist with survey readiness and regulatory compliance efforts.
