Qualifications

• Must hold a state license/certification in social work (where practice is required).
• Bachelor’s degree in Social Work or related Human Services field.
• Minimum of 1 year of social work experience in a healthcare or geriatric setting.
• SNF experience preferred; nursing SNF experience a plus.

Responsibilities

• Conduct psychosocial assessments and contribute to individualized care plans.
• Support residents and families through transitions into long-term care and memory care.
• Complete MDS-related documentation accurately and in compliance with regulations.
• Advocate for resident rights, dignity, and self-determination.
• Coordinate discharge planning and connect residents with community resources.
• Collaborate with nursing, administration, and the interdisciplinary team.
• Assist with survey readiness and regulatory compliance efforts.
