The role involves assessing emotional and behavioral issues, participating on an interdisciplinary team, and aiding in treatment planning. Responsibilities include discharge planning, care planning conferences, coordinating with community resources, supervising interns, and developing staff education.

Qualifikationen

• Requires a bachelor’s degree in social work and at least one-year experience in a social service program for the elderly or related field.
• Must be licensed (full or limited) as a bachelor’s level clinical social worker with the State of Michigan.

Aufgaben

• Assess and evaluate residents’ psychosocial needs and develop goals for providing the necessary service and participate in admission process as needed.
• Incorporate social service goals in the resident’s plan of care and attend care planning conferences.
• Assist residents and families to utilize community resources when not provided directly by the corporation.
• Maintain confidential records and interviews with residents and families as appropriate.
• Provide supervision for social work student interns.
• Assist in development, supervision and education of staff.
• Serve as the team lead in discharge planning.
• Attend Clinical IDT meetings and advocate for resident rights.
