Job summary

Level I requires a human services degree and one year of relevant experience, or a nursing license with one year of related work.

Qualifications

• Bachelor’s degree in a human services field or related degree. (*)
• One year of professional experience with individuals with IDD or in social work/case management.
• For Level II: two years of professional experience with the same populations, or nursing license with two years of related experience.

Responsibilities

• Under supervision, provide services to individuals with IDD living at home or in community arrangements; assess histories and coordinate resources to meet consumer needs.
• Meet compliance with Federal Home and Community Based Waiver regulations, state and policy guidelines.
