Job summary

Avamere Rehab of Newport, located in Newport, Oregon, is seeking a Social Services Coordinator to oversee psychosocial care needs under the Social Services Director. The role includes coordinating discharge planning and serving as a liaison among residents, families, and community partners.

The coordinator will work with home health, hospice, and payors to ensure post-discharge supports, manage care conferences, and advocate for residents' rights and well-being in a collaborative,

Qualifications

• Bachelor's degree in public health administration or social work or a related field.
• Experience with electronic medical records and computer documentation systems.

Responsibilities

• Identifies residents' emotional, mental, and psychosocial needs and ensures that comprehensive care plans are in place and up to date.
• Coordinates with the Social Services Director and interdisciplinary team to plan safe discharges from the facility.
• Communicates with residents, families, community providers and programs on a regular basis.
• Advocates for residents' rights and ensures actions are in the resident's best interest.
