Job summary

Pikes Peak Post Acute is seeking an Activity Assistant to support the Activities Director in delivering a creative, engaging program aligned with resident needs and regulatory standards.

You will help plan and conduct activities for individuals and groups, coordinate calendars, and assist with assessments, transportation, and documentation while maintaining a safe, welcoming environment for residents and families.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between employees, residents, families, and the public.
• Assist in community planning related to resident interests.
• Develop monthly activity calendar and maintain attendance records.
• Assist QA/Assessment Committee with action plans for quality improvements.
• Assist with discharge planning and activity care plans.
• Arrange transportation for residents to outings, appointments, or discharge.
• Develop monthly activity schedules including outings, group activities, and in-room activities for bed-bound residents.
• Encourage resident participation in hobbies, crafts, and reading; provide materials (Braille or audio books as appropriate).
• Assist with assessment documentation and quarterly progress notes.
• Keep Activity Department clean, orderly, and secured.
