The ideal candidate has a high school diploma or equivalent, a valid Indiana driver’s license, and completed an approved Activity Program Training Course (or qualifies as a TRS/OT/OTA).

Qualifications

• Completed State-approved Activity Program Training Course or equivalent (e.g., TRS, OT/OTA).

Responsibilities

• Assess residents on an individual basis and formulate care plans.
• Develop an activity program appropriate to residents' backgrounds and needs.
• Develop work assignments for activities personnel.
• Advise Administrator on staffing needs and supplies/equipment for activities.
