Job summary

Chateau Nursing and Rehab in Indiana is seeking an experienced Activity Director to elevate daily living experiences for residents and patients. You will design engaging activities, collaborate with care teams and support staff to foster social interaction and well-being.

Responsibilities include developing ongoing programs, maintaining records, creating calendars, leading volunteers, and participating in care plan meetings.

Qualifications

• High School Diploma.
• 1 - 2 years' experience in a social or recreational program in a long term setting is preferred.

Responsibilities

• Develop an ongoing program for group and individual activities based on residents' needs, interest, and abilities as identified in assessment / care plans.
• Participate in care plan meetings.
• Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy.
• Create the monthly activity calendar.
• Provide training and in-services.
• Implement and maintain established policies and procedures.
• Guide, lead and provide motivation for participation in Resident Council, schedule monthly meetings.
• Lead, establish, supervise and maintain the volunteer program.
