Job summary

Willow Creek Health Care Center, LLC is seeking a compassionate Activities Assistant to support the Activity Director in delivering a meaningful, interactive activities program for residents in a full-time role.

You will help plan and conduct activities, coordinate transportation, and assist with care plans while adhering to federal, state, local, and corporate standards. This position emphasizes resident quality of life and engagement.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in maintaining communication between staff, residents, families, and community partners.
• Develop monthly activity calendars and track attendance.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents to outings, appointments, or discharge.
• Create monthly activity schedules including outings and in-room activities for bed-bound residents.
• Encourage participation in hobbies and reading, providing Braille or audio materials as needed.
• Assist with assessment documentation and quarterly progress notes.
• Keep the Activity Department clean, orderly, and secure.
