Job summary

Willoughby Health Center is seeking an Activities Assistant (Healthcare) to help plan and conduct individual and group activities, and to support residents, families, and staff with clear communication.

The role includes developing a monthly activity calendar, tracking attendance, assisting with assessment documentation, coordinating transportation, and encouraging participation in hobbies and reading.

Qualifications

• High school diploma or equivalent.
• Preferable one year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals.

Responsibilities

• Plan and conduct activities for individuals and groups.
• Assist in communication between staff, residents, families and the public.
• Develop monthly activity calendar and maintain attendance.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents as needed.
• Encourage participation in hobbies, crafts and reading.
• Assist with assessment documentation and progress notes.
• Keep the activity department clean, orderly and secured.
