Job summary

Watsonville Post Acute is seeking an enthusiastic Activity Assistant to support resident-centered programs and care plans. You will plan, organize, and conduct activities that meet residents’ needs, and coordinate with family, staff, and the charge nurse to ensure respectful, dignified engagement.

Responsibilities include documenting progress notes, arranging field trips as needed, and adhering to HIPAA, infection control, and facility policies.

Qualifications

• High school diploma or GED required.
• Must be able to read, write, speak, and understand the English language.
• Must be able to follow directions.

Responsibilities

• Plan, organize and conduct activities for the facility.
• Involve residents and family members in planning where possible.
• Arrange transportation for field trips when necessary.
• Ensure all activity progress notes are descriptive and detailed.
• Coordinate activities with other departments and work collaboratively with staff.
• Follow Infection Control policies and Safety and Sanitation procedures.
• Report occupational exposures to a supervisor immediately.
• Protect resident files and data with HIPAA confidentiality.
• Abide by facility policies including not disclosing user IDs and passwords.
