AuthentiCare Health Centers is seeking an Activities Assistant to support residents in long-term, sub-acute and memory care settings. You will work with the Activities Director to plan and run engaging programs, tailor activities to individual residents, and help maintain a warm, resident-centered environment.

You will support planning calendars, document plans of care, and participate in discharge planning. The role emphasizes compassion, teamwork, and reliable attendance in a caring facility.

Formación

• High school diploma or equivalent.
• Experience in long-term care or healthcare settings is a plus.
• Experience working with Alzheimer's/Dementia patients is a plus.
• Experience in Long Term Care.
• Must be capable of maintaining regular attendance.
• Good verbal and written communication skills.
• Must be able to maintain confidentiality regarding employee, resident and company proprietary information.
• Must relate professionally with patients, families and staff.
• Must possess computer skills necessary for the job.

Responsabilidades

• Assist Activities Director in enhancing residents' intellectual, emotional and spiritual interests with activities.
• Plan, develop, organize, implement and evaluate activity programs.
• Develop monthly program calendar.
• Create written plan of care for each resident's activity program.
• Participate in discharge planning and resident assessments for activity care plans.
• Implement activity procedures as directed by the Activity Director.
• Schedule programs, plan events and provide games/activities for residents.
• Encourage participation in activities that stimulate residents physically, mentally, socially and spiritually.
• All other duties as assigned.
