Job summary

Pikes Peak Post Acute is seeking an Activity Assistant to support the Activity Director in running a creative and interactive activities program. The role focuses on meeting residents' physical, mental, and psychosocial needs within a long-term care facility.

Responsibilities include planning and conducting activities, maintaining calendars and records, coordinating transportation, and assisting with discharge planning.

Qualifications

  • Education: High school diploma or equivalent; preferable one-year experience in a long term care facility.
  • Language Skills: Ability to read technical procedures and policies; communicate effectively with managers, employees, residents and families.
  • Mathematical Skills: Apply fractions, percentages, ratios, and proportions to practical tasks.
  • Reasoning Ability: Solve practical problems and follow diverse instructions.
  • Certificates: N/A

Responsibilities

  • Participate in planning and conducting activities for individuals and groups.
  • Assist in communication between staff, residents and families to meet residents' needs.
  • Contribute to community planning related to facility interests and resident needs.
  • Develop monthly activity calendars and maintain attendance records.
  • Assist in quality improvement actions as directed by the Activities Director.
  • Assist in discharge planning and develop activity care plans.
  • Arrange transportation for residents as needed for outings, appointments, or discharge.
  • Encourage resident participation in hobbies, crafts and reading; provide Braille or audio books as needed.
  • Maintain documentation including assessment records and progress notes.
  • Keep the Activity Department clean, orderly and secure.
