PACS is seeking a Reserved Bed Post-Acute Care Manager to serve as the liaison between hospital partners and post-acute care centers. You will coordinate referrals, verify eligibility, manage patient placement, and report KPIs to ensure seamless transitions and contractual compliance.
This remote role requires strong communication with hospital case management, experience in admissions, and ability to travel for onsite assessments.
The Reserved Bed Post-Acute Care Manager serves as the primary liaison between hospital partners and PACS post-acute care centers, facilitating seamless patient transitions through the Reserved Bed Program. This role is responsible for coordinating referrals, managing patient placement, maintaining program metrics and reporting, and fostering strong relationships with hospital case management teams. The manager plays a critical role in supporting program growth, ensuring contractual compliance, and delivering outstanding service to both hospital partners and PACS facilities.
Must possess, as a minimum, a high school diploma or GED. Bachelor’s degree in health care or related field preferred. Two (2) years of Admissions experience preferred. LVN or RN license required.
Must maintain a professional, quiet, and distraction-free remote work environment that supports productivity, confidentiality, and effective job performance. Remote work is not a substitute for childcare or dependent care. Employees must have appropriate childcare or caregiving arrangements in place during scheduled work hours and be able to devote their full attention to job responsibilities. Employees may also be required to travel to hospital locations to conduct onsite assessments and perform other business-related duties as needed.
To perform this job successfully, an individual must be proficient in Microsoft Suite products and hospital EMAR systems.
