Job summary

Barnes-Jewish Hospital is seeking an experienced transplant coordinator to manage care for solid organ transplant patients and donors. You will coordinate with physicians and the health care team, providing education and discharge planning across the transplant continuum.

The role requires a BSN or nursing diploma, RN licensure, and 2–5 years of related experience. Onsite, daytime schedule; transplant experience is a plus and preferred.

Qualifications

• Nursing degree or diploma required; RN licensure expected.
• Experience coordinating care for transplant or similar patients is preferred.
• Strong communication and teamwork skills essential for a multidisciplinary team.

Responsibilities

• Manages the health care of transplant patients and donors.
• Coordinates care with physicians and health care team.
• Provides education throughout continuum of care.
• Documents patient assessment, plan of care and teaching.
• Collaborates with interdisciplinary team to identify and resolve problems in patient care management.
