Job summary

Barnes-Jewish Hospital at Washington University Medical Center is seeking a transplant coordinator to manage care before and after solid organ transplantation. The role coordinates data, assesses suitability of recipients and donors, and provides discharge instructions and follow-up planning.

You will work with physicians and the care team to monitor outcomes, educate families, and support rehabilitation across the transplant continuum in a Magnet-certified hospital environment.

Qualifications

• RN with Nursing Diploma/Associate's degree
• Bachelor's Degree in Nursing preferred
• 2-5 years transplant-related experience; 5-10 years preferred
• RN license required; Cert Clinical Transplant Coord preferred

Responsibilities

• Manages the health care of transplant patients and donors.
• Coordinates care with physicians and health care team.
• Provides education throughout continuum of care.
• Documents patient assessment and plan of care, implementation and evaluation.
• Collaborates with interdisciplinary team to resolve problems in patient care management.
