Qualifications

Responsibilities

• Perform clinical duties in designated renal services area with comprehensive exams and plans.
• Record histories, exams, diagnoses, and care recommendations clearly and timely.
• Educate patients and families to support self-management and follow-up.
• Collaborate with interdisciplinary team to ensure high-quality outcomes.
• Stay updated on practice trends and participate in PHHS education activities.
• Maintain compliance with applicable state/federal rules and organizational policies.
• Identify process improvements to enhance efficiency and patient experience.
• Support department goals and attend regular staff meetings.
