Job summary

Concerto Renal Services is seeking a Renal Dietitian to join our team in the Greater Baltimore area. The role focuses on providing nutrition education, counseling, and interventions for End Stage Renal Disease (ESRD) patients and caregivers in clinical settings to maximize nutritional well-being.

The Renal Dietitian will independently evaluate nutrition, develop and update care plans with the IDT, and monitor patient status to adjust interventions as needed, documenting progress monthly.

Qualifications

• Nutrition assessment and evaluation skills.
• Interdisciplinary collaboration with IDT (RN, RD) and physicians.
• Education and counseling of ESRD patients and caregivers.
• Development of Medical Nutrition Therapy plans.

Responsibilities

• Independently evaluates and documents nutrition assessment and reassessment per CMS regulations.
• Collaborates with patient, IDT, and physicians on plan of care and lab considerations.
• Develops individualized medical nutrition therapy based on labs and goals.
• Provides nutrition education and counseling tailored to patient needs and circumstances.
• Documents monthly nutrition progress notes and annual reassessments.
