Concerto Renal Services is seeking a Renal Dietitian to join our team in the Greater Baltimore area. The role focuses on providing nutrition education, counseling, and interventions for End Stage Renal Disease (ESRD) patients and caregivers in clinical settings to maximize nutritional well-being.

The Renal Dietitian will independently evaluate nutrition, develop and update care plans with the IDT, and monitor patient status to adjust interventions as needed, documenting progress monthly.

Qualifications

• Nutrition assessment and evaluation skills.
• Interdisciplinary collaboration with IDT (RN, RD) and physicians.
• Education and counseling of ESRD patients and caregivers.
• Development of Medical Nutrition Therapy plans.

Responsabilités

Summary/Objective

Concerto is seeking a Renal Dietitian to join its team! The Renal Dietitian is responsible for providing nutrition education, counseling and interventions as appropriate to End Stage Renal Disease (ESRD) patients and authorized caregivers in clinical settings to maximize the patients nutritional well-being.

Essential Functions

• Independently evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with CMS regulations.
• Works with patient and IDT to establish, document and update patients plan of care (POC).
• Assesses ongoing nutritional status to determine appropriate nutrition interventions and recommendations. Consults with ECF staff (RN, RD) and physicians regarding lab tests and medications adjustments as needed.
• Develops individualized Medical Nutrition Therapy for patients based on reported lab results and individualized goals.
• May request assistance from the lead dietitian as needed with advanced or complex clinical renal nutrition situations.
• Provides nutrition education and counseling based on individualized patient needs including nutrient requirements, knowledge and abilities, medical, economic and social situation.
• Selects and develops education materials and methods appropriate for individual patient.
• Monitors nutritional status, laboratory values, dialysis adequacy, adherence to diet and response to nutrition related medications and therapy to determine appropriate nutrition intervention and recommendations.
• Documents nutrition progress notes at least monthly.
• Conduct nutrition assessment upon patient admission, 90-day assessment and annually.
