Job summary

aQua Dialysis Clear Lake is seeking a full-time Master's Level Social Worker to join the treatment team at our dialysis center. You will support patients and families adapting to ESRD and act as an advocate within a multi-disciplinary team including nephrology, nursing, and dietetics.

Key duties include evaluating vocational rehab options, providing education and referrals, assisting with insurance coverage, rights education, supportive counseling, and Advanced Directives education.

Qualifications

• Master's degree in Social Work is required.
• State license is preferred.
• Unlicensed applicants must obtain a temporary license within three months of employment.
• Prior experience in dialysis, hospital or other medical setting is preferred.

Responsibilities

• Evaluate vocational rehabilitation options (employment, school, volunteering, and activities).
• Provide education and referrals to appropriate resources.
• Help patients keep or obtain insurance coverage.
• Assist patients in understanding their rights and responsibilities.
• Provide supportive counseling to patients and families.
• Inform patients about the importance of treatment participation.
• Provide Advanced Directives education.
