aQua Dialysis Fort Bend is seeking a full-time Master's Level Social Worker to support patients and families adjusting to ESRD in our dialysis center. You will advocate for patients and coordinate with the treatment team to address needs, rights, and participation in care.

Responsibilities include counseling, education, referrals, insurance assistance, and Advanced Directives education. A Master's in Social Work is required; state license preferred. Full-time benefits are offered.

Qualifications

• Master's degree in Social Work is required.
• State license is preferred; unlicensed applicants must obtain temporary license within 3 months.
• Prior experience in dialysis, hospital, or medical setting is preferred.

Responsabilités

• Evaluate vocational rehabilitation options (employment, school, volunteering, activities).
• Provide education and referrals to resources.
• Help patients maintain or obtain insurance coverage.
• Assist patients in understanding their rights and responsibilities.

aQua Dialysis Fort Bendis seeking a full-time Master's Level Social Worker to be a key member of the treatment team for patients in our dialysis center.

As a dialysis / renal social worker, you will provide support for the patient and family who are adjusting to the challenging and overwhelming diagnosis of end stage renal disease (ESRD). You will be the patient's advocate and serve as a bridge in communicating the patient's needs to the treatment team members, which consist of the nephrologist, nurse, dietitian, patient, and of course the social worker.

In addition to assisting patient with learning how to cope with kidney disease and adjusting to the life changes of dialysis, other social worker responsibilities include:

• Evaluating for vocational rehabilitation services which includes employment, going to school, volunteering within the community, or returning to previously enjoyed activities
• Providing education and referrals to appropriate resources
• Assisting with keeping or obtaining insurance coverage
• Assisting patients with understanding their rights and responsibilities
• Providing supportive counseling
• Assisting in informing patients of the importance of treatment participation
• Advanced Directives education.

A Master's degree in Social Work is required. State license is preferred. Unlicensed applicants must be able to obtain temporary license within the first three months of employment. Prefer prior experience in Dialysis, Hospital or other medical setting preferred.
