Job summary

University Hospitals of Leicester NHS Trust is seeking a Staff Nurse to support renal care delivery within the Renal Community Team. The role involves caring for a delegated caseload of patients, including home dialysis training and patient education.

You will assess, plan, implement and evaluate care, coordinate nursing activities, supervise colleagues and work with a multidisciplinary team to ensure high-quality care and safe discharge.

Responsibilities

• Assess, plan, implement and evaluate evidence-based nursing care for a defined patient group.
• Assist in management and organisation of the Renal Community Team in clinical and community settings.
• Provide home dialysis training and support for patients performing PD or HHD.
• Contribute to education of chronic kidney disease patients.
• Coordinate, supervise and develop the nursing team on a day-to-day basis.
• Maintain accurate nursing records and report patient information.
• Collaborate with multidisciplinary teams to meet care needs and safe discharge.
• Promote health equality in all practice areas.
• Act as a professional and clinical role model to staff, patients and the public.
• Maintain effective communication with patients and carers.
