Ez Health is recruiting Wellness Coaches to assist older adults in navigating health care, benefits, and daily needs by phone. You will be a consistent, caring point of contact, listening closely and guiding clients to clear next steps.

This role emphasizes relationship-building with a small client load and long-term engagement. Responsibilities include regular check-ins, uncovering barriers, preparing clients for appointments, and coordinating with doctors, pharmacies, insurers, and community

Qualificações

• Excellent listening skills and ability to understand client needs.
• Comfort with coordinating and following through on tasks.
• Strong verbal communication over the phone.
• Capable of staying organized with multiple client files.

Responsabilidades

• Reach out to clients regularly to monitor well‑being and progress.
• Ask targeted questions to uncover barriers and needs.
• Help clients prepare for appointments and follow up afterwards.
• Explain benefits, referrals, and next steps clearly.
• Coordinate with doctors' offices, pharmacies, insurers, and community groups.
• Arrange transportation, meals, prescription help, and local programs for clients.
• Maintain organized notes for team visibility.
• Follow through on every task to ensure clear outcomes.
