Job summary

You will optimize pre-hospitalization interventions and post-acute transitions to improve member health and cost performance.

The VP will guide a large, geographically distributed team, ensure regulatory alignment, and drive measurable improvements in patient outcomes while reducing readmissions and gaps in care.

Qualifications

• 5+ years of progressive leadership in care management, population health, or clinical operations.
• Strong understanding of Medicare Advantage regulations and risk adjustment.
• Experience building scalable clinical programs with measurable outcomes.

Responsibilities

• Develop and execute a comprehensive clinical strategy aligned with cost and quality goals.
• Oversee end-to-end clinical program lifecycle from concept to scaling.
• Lead cross-functional teams and partner with providers, networks, and vendors.
