Job summary

Baptist Health is seeking a Remote Value Based Care Pharmacy Technician to join our Pharmacy team. You will identify at-risk patients, conduct outreach to improve adherence, and coordinate medication-related services within a population health framework.

Under pharmacist supervision, you will leverage EHRs and payer data to close care gaps, support transitions of care, and help achieve quality metrics such as Medicare Star Ratings and HEDIS.

Qualifications

• High school diploma or equivalent.
• Pharmacy Technician registration with Kentucky Board of Pharmacy upon hire.
• Previous retail, mail order, central/call center, value based care pharmacy work experience preferred.
• National pharmacy technician certification (PTCB) preferred.

Responsibilities

• Identify patients at risk for nonadherence using payer reports and EHR data.
• Conduct outbound patient outreach to improve adherence and address barriers.
• Coordinate medication refill reminders, synchronization, and home delivery.
• Escalate clinical concerns to pharmacists or providers when issues arise.
• Support transitions of care by ensuring discharge meds and proper regimens.
• Document patient interactions in the electronic health record and monitor outcomes.
• Monitor Medicare Star Ratings, HEDIS, and other quality measures.
• Collaborate with care teams to coordinate patient care and improve access.

Education
