Job summary

Yale NewHaven Health is seeking a Transitional Care Manager Triage RN to support post-discharge triage, follow-up, and care coordination. The role focuses on safe transitions of care, reducing unnecessary ED visits and readmissions, and guiding patients to appropriate levels of care with education and coordinated referrals.

Qualifications

• Bachelor's degree in clinically related field and RN licensure.
• Minimum 3 years of direct patient care experience; 2 years in case management.
• Connecticut RN licensure required and ability to travel to sites as needed.

Responsibilities

• Perform post-hospital discharge and post-ED outreach to reduce readmissions.
• Coordinate referrals and review discharge plans and medications.
• Document in EHR, generate reports, and ensure quality and patient experience standards.
• Collaborate with care management teams to enroll patients in CCM when eligible.
