Job summary

Affinity Hospice is seeking a Centralized Triage Registered Nurse (RN) to provide remote, telephone-based nursing care for hospice patients and families. You will serve as the primary contact during after-hours, weekends, and holidays across multiple service areas, guiding symptom management and coordinating with the field team.

The role requires an active RN license (multistate preferred) and CPR certification, with 2+ years in hospice/palliative or related critical care.

Qualifications

• Associate’s degree in nursing is required; a Bachelor's degree is preferred.
• Two years of hospice or palliative care experience is preferred.
• Experience in telephone triage or emergency care is preferred.
• Active RN license with multistate (compact) license is required.
• Current CPR certification is required.

Responsibilities

• Triage incoming calls and provide clinical guidance by phone.
• Coordinate after-hours/holiday coverage with hospice team.
• Document triage interactions and nursing assessments in EMR.
• Dispatch hospice field staff for in-person visits when needed.
• Educate patients and families on end-of-life care and symptom management.
