Job summary

Guideway Care is seeking an experienced and compassionate Registered Nurse to join our remote triage team. This RN will provide high-quality assessment, guidance, and care coordination to patients by phone and secure messaging, working remotely in a structured environment.

Ideal candidates have an active RN license, CA licensure, and at least five years of direct patient care across ED, L&D, critical care, or women's health, with remote triage experience highly preferred.

Qualifications

• Active, unencumbered RN license; compact licensure state.
• California RN licensure is required.
• Minimum 5 years' direct patient care in multiple settings.
• Remote RN triage or telephone-based care experience preferred.
• Strong clinical judgment, problem-solving and decision-making.
• Excellent verbal and written communication, empathetic delivery.
• Proficient with EMRs (Epic, Cerner, Greenway) and standard software.
• Ability to work independently within clinical protocols.

Responsibilities

• Conduct thorough clinical assessments and triage per protocols.
• Respond to patient inquiries via calls and secure messaging.
• Provide care recommendations or escalate as needed.
• Document assessments and care coordination in EMRs.
• Support administrative functions related to patient care and documentation.
• Assist training and onboarding of team members.
• Collaborate with healthcare providers to ensure continuity of care.
