Job summary

Neighborhood Healthcare is seeking a Registered Nurse for Telehealth Triage on a full-time, temporary basis. The role supports patient triage via telephone and digital channels, coordinating care and referrals while maintaining strict patient confidentiality.

The position requires an Associate's degree in nursing (Bachelor's preferred), CA RN license, and a current BLS certification. Expect remote work with occasional training at Escondido or Moreno Valley HQ, and a 8-5 schedule Monday–Friday.

Qualifications

• Associate's degree in nursing required; Bachelor's degree in nursing preferred.
• CA RN license required.
• Current BLS certification through an American Heart Association-approved source is required upon hire and must be maintained as a condition of employment.
• Three years’ nursing experience; nursing telephone assessment and triage experience preferred.
• Valid CA driver’s license or out of state driver’s license for active military personnel/spouses required; proof of auto insurance.

Responsibilities

• Answers patient calls on a wide range of healthcare topics during normal clinic hours.
• Assesses patients understanding of their condition and identify potential care and/or provider gaps.
• Evaluates, assesses, and triages immediate health concerns and documents outcomes.
• Provides patient advice and education on handling their situation.
• Provides ongoing training and support to Call Center Representatives and others to assure effective patient services.
• Serves as a clinical resource to the Call Center and across the continuum of care to influence standards related to patient safety and care delivery.
