Elevance Health is seeking a Telephonic Nurse Case Manager II to manage care for members with complex needs. This is a remote-friendly role with required in-person training sessions and occasional office visits, aligning with our hybrid work approach.

The role requires a BA/BS in a health-related field, an active RN license, and multi-state licensure if serving multiple states. You will assess, develop, implement and monitor care plans, coordinating resources and communicating with Medical

Competenze

• Requires BA/BS in a health-related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in applicable state required.
• Multi-state licensure is required if this individual is providing services in multiple states.

Mansioni

• Care management for members with complex and chronic needs, telephonically assessed and managed.
• Develops, implements, coordinates, monitors, and evaluates care plans to optimize health care across the continuum.
• Facilitates authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Coordinates internal and external resources to meet identified needs.
• Monitors and evaluates the care management plan and modifies as necessary.
