Qualifications

• Requires BA/BS in a health-related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in applicable state required.
• Multi-state licensure is required if providing services in multiple states.

Responsibilities

• Ensure access to services aligned with member health needs.
• Assess and develop care management plans with objectives and goals.
• Coordinate authorizations/referrals within benefits or extra-contractual arrangements.
• Coordinate internal and external resources to meet identified needs.
• Monitor outcomes and adjust care plans as needed.
• Interface with Medical Directors/Physician Advisors on treatment plans.
