Qualifications

Responsibilities

• Assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care across the care continuum; telephonically.
• Perform duties telephonically.
• Ensure member access to services and coordinate care across the continuum.
• Interface with Medical Directors and Physician Advisors on care plans.
• Coordinate authorizations/referrals within benefits structure or through extra-contractual arrangements.
