The role supports a hybrid/remote work model with in-person training as needed. Responsibilities include assessing needs, coordinating care plans, facilitating authorizations, and collaborating with medical directors to optimize outcomes.

Qualifications

• Requires BA/BS in a health-related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in applicable state required.
• Multi-state licensure is required if providing services in multiple states.

Responsibilities

• Assess, develop, implement, coordinate, monitor and evaluate care plans telephonically.
• Ensure access to services appropriate to health needs and coordinate authorizations/referrals.
• Monitor effectiveness of care plans and modify as needed.
• Collaborate with Medical Directors and Physician Advisors on treatment plans.
• Utilize Microsoft Office and related systems; respond to messages timely.
