Elevance Health is seeking a Telephonic Nurse Case Manager II to manage complex care plans for members across states. This role performs care coordination and telephonic assessments, ensuring access to appropriate services and timely authorizations.

You will work within a hybrid/virtual framework, requiring multi-state licensure and a current RN license. Strong critical thinking and communication are essential for success.

Qualificações

• Requires BA/BS in a health-related field or equivalent experience.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure is required for services in multiple states.

Responsabilidades

• Assess, develop, implement, coordinate, monitor and evaluate care plans telephonically.
• Ensure access to services aligned with member health needs.
• Facilitate authorizations/referrals within benefits structure.
• Coordinate internal/external resources to meet identified needs.
• Monitor and adjust care plans based on effectiveness.
• Collaborate with Medical Directors and Physician Advisors on care plans.
