Elevance Health seeks a Telephonic Nurse Case Manager II to manage complex patient care telephonically from California with multi-state licensure. The role requires RN licensure in California and 5+ years of clinical experience, plus the ability to assess and coordinate care across the continuum.

The position offers a hybrid/virtual work environment with in-person onboarding and occasional training sessions, and a salary range that reflects market rates for telephonic case management.

Qualifications

• Requires BA/BS in a health related field or equivalent experience.
• Current, unrestricted RN license in California is required.
• Multi-state licensure is required if serving multiple states.

Responsibilities

• Assess, develop, implement, coordinate and evaluate care plans telephonically.
• Ensure access to services and coordinate authorizations/referrals.
• Monitor effectiveness of care plans and adjust as needed.
• Interface with Medical Directors and Physician Advisors on plans.
• Assist with problem solving with providers, claims or service issues.
• Develop utilization/care management policies and procedures.
