Job summary

Elevance Health is seeking a Telephonic Nurse Case Manager I to perform care management for members with complex needs, mostly telephonically. The role offers flexible, remote work with required in-person training, and occasional location alternatives within commuting distance.

Qualifications

• Requires BA/BS in a health-related field or equivalent experience.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure required for multi-state service.

Responsibilities

• Perform telephonic care management within licensure scope for complex chronic needs.
• Assess, develop, implement, coordinate, monitor, and evaluate care plans.
• Facilitate authorizations/referrals within benefits or extra-contractual arrangements.
• Coordinate internal and external resources to meet needs.
• Monitor effectiveness of care plans and adjust as needed.
• Interface with Medical Directors and Physician Advisors on care plans.
• Negotiate rates of reimbursement as applicable.
• Assist in problem solving with providers, claims or service issues.
