Job summary

AMC Health is seeking a Telephonic Nurse Care Manager to centrally manage patients with chronic and complex conditions via telephonic and remote assessment. The role partners with patients, caregivers and interdisciplinary teams to implement evidence-based plans, monitor data, and proactively reduce risk in a fast-paced virtual care environment.

Qualifications

• Nursing degree from an accredited college or university (ADN or BSN)
• 5+ years of broad clinical nursing experience
• 3+ years in care management, home health, telehealth, or chronic disease management

Responsibilities

• Conduct comprehensive telephonic and virtual clinical assessments with trend analysis of biometric data
• Develop and adjust individualized nursing care plans
• Engage patients using Motivational Interviewing techniques
• Identify clinical risks and escalate per guidelines
• Collaborate with interdisciplinary care teams to coordinate care and transitions
• Deliver interventions to reduce hospitalizations and costs
• Document patient interactions in the EHR accurately
• Participate in quality improvement, chart audits, and performance reviews
• Perform care management aligned with client workflows and SLAs
• Maintain compliance with clinical, regulatory, HIPAA, and organizational standards
