Job summary

AMC Health is seeking a Telephonic Nurse Care Manager to remotely manage patients with chronic and complex conditions. You will conduct telephonic assessments, interpret biometric data, and develop evidence-based care plans in collaboration with interdisciplinary teams.

Ideal candidates hold an RN license (ADN/BSN) with 5+ years of nursing and 3+ years in care management or telehealth, plus strong communication and EHR skills.

Qualifications

• RN with ADN or BSN
• 5+ years broadly in clinical nursing
• 3+ years in care management, home health, or telehealth
• Strong computer proficiency with EHRs and multiple systems
• Able to work across time zones and schedules based on client needs

Responsibilities

• Conduct telephonic and virtual clinical assessments with data interpretation
• Develop and adjust individualized nursing care plans
• Engage patients using Motivational Interviewing to drive adherence
• Identify risks and escalate per clinical guidelines
• Collaborate with care teams to coordinate referrals and transitions
• Deliver interventions to reduce hospitalizations and cost of care
• Accurately document interactions in the EHR
• Participate in quality improvement initiatives
