Job summary

Qualifications

Responsibilities

• Assess patients in an ALF/SNF environment.
• Provide palliative and pain management to treat the patient best.
• Perform pain management procedures as needed.
• Define and document patient diagnoses.
• Prescribe and refill medications.
• Patient and family education and management.
• Notate findings and treatment course in patient charts, including follow-up notes.
• Actively participate in departmental activities to improve training methods and achieve company goals.
