Job summary

HarmonyCares is seeking a Nurse Practitioner for a PRN Telehealth role in Kentucky. The NP will perform annual risk assessments remotely, within the NP scope of practice as delegated by the Collaborative Physician.

The position involves in-home and telehealth assessments, documentation to NCQA standards, and coordination with PCPs, patients, and care teams. A valid NP license, board certification, BLS, and EHR skills are required.

Qualifications

• Active/unrestricted nurse practitioner license in coverage states.
• Board certification by ANCC, AANP, or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Valid and unencumbered driver's license with good driving record.
• Outstanding EHR skills.

Responsibilities

• Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and address patient needs.
• Take patient history, perform examinations, and determine diagnoses.
• Provide written documentation of visits per NCQA standards.
• Record vital signs as needed and arrange case management referrals as appropriate.
• Communicate with patients, caregivers, agency nurses, providers, and vendors to ensure accurate diagnosis.
• Maintain confidentiality and observe OSHA safety practices.
• Engage in meetings, training, and professional development as requested.
