Job summary

Optum Home & Community Care is seeking a Telehealth National On-Call Advanced Practice Clinician (APC) to provide telephonic and telehealth care 24/7 across multiple markets. This remote, part-time role requires evenings, nights, and rotating weekends, with a commitment of 20 hours per week and a CT or RI NP license.

You will use EMR daily, coordinate with care teams, and ensure high-quality, compliant care delivery while maintaining patient privacy and security in a remote setting.

Qualifications

• Master of Science in Nursing or Doctor of Nursing Practice required.
• Active and unrestricted NP license in CT or RI required.
• Board certification in family, adult, gerontology or acute care NP.
• Current DEA licensure/prescriptive authority or ability to obtain.
• Proficient computer skills including electronic medical records.

Responsibilities

• Provide telephonic/telehealth care and direction to patients, caregivers, and staff for 24/7 coverage.
• Deliver care across care settings, including medical and behavioral health with physician collaboration.
• Document encounters and care plans compliant with CMS regulations.
• Coordinate care across transitions and manage plan adjustments as needed.
