Circadia Health is seeking a full-time Nurse Practitioner to conduct remote assessments and coach care teams using the Circadia Intelligence Platform. The role is hybrid: three days of home visits and two days of telehealth for home health patients in WA.

You will interpret predictive signals, ensure timely interventions, and document findings on the same day. Strong telehealth and independent practice are essential for success.

Formación

• Must be licensed NP in WA with ANCC or equivalent.
• 2+ years NP experience in home health or post-acute care.
• Comfortable with telehealth platforms and remote assessments.

Responsabilidades

• Conduct remote telehealth assessments and follow-ups for home health patients.
• Interpret Circadia data trends to prioritize high-risk patients.
• Complete documentation/charts within 24 hours of encounters.
• Manage 25+ tele-visits per day for designated home health patients.
• Provide feedback to improve virtual workflows and integrations.
