Circadia Health in California is seeking a full-time Nurse Practitioner to provide hybrid care for home-health patients, combining telehealth and in-home visits. You will conduct remote assessments, act on predictive health signals, and guide care teams to intervene earlier, reducing hospitalizations.

The role includes three days of in-home visits and two days of telehealth, with a strong focus on documentation, patient safety, and clinical collaboration to improve outcomes for seniors.

Qualifications

• Telehealth experience and ability to conduct remote assessments.
• Independent clinical practice in home-health or post-acute care.
• Strong English communication for patient and team coordination.

Responsabilités

• Perform remote telehealth assessments for home-health patients and adjust plans of care.
• Interpret Circadia platform insights and prioritize high-risk patients for timely interventions.
• Complete documentation within 24 hours of each encounter.
• Manage 25+ tele visits per day for designated patients in SoCal/OC.
• Provide mentorship and onboarding support to NP colleagues and collaborate with clinical ops/product teams.
