Job summary

VITAS Healthcare is seeking an RN for Telecare after-hours coverage. The role focuses on assessing physical, psychological, social, and spiritual needs and initiating interventions with the nursing process and Telecare Protocols.

You will collaborate with physicians, facility staff, case managers, and external vendors to coordinate care and ensure quality outcomes. Evening, night, weekend shifts may be required, with licensure in multiple states as needed.

Qualifications

• Graduate from an accredited school with an Associate or Bachelor’s degree in Nursing.
• Two years’ experience in acute-care hospital nursing in medical-surgical, oncology, home health, or emergency settings.
• Live within 75 mile radius of Phoenix, AZ; eligible for licensure in other states where VITAS Triage services operate.

Responsibilities

• Provides clinical assessment and intervention using the nursing process and Telecare Protocols.
• Acts as patient/family advocate, coordinating with physicians, facility staff, case managers and external resources.
• Coordinates after-hours service delivery, including education, dispatching interdisciplinary visits, and external coordination.
